Sometimes a Great Notion

I read this many years ago, and just finished it again. The one scene that stuck with me, and drew me back, is the scene where:

Trigger Warning!!!1! :

someone actually dies laughing

Now, reading it again, I'm really impressed with how Kesey wove together so many backstories, sometimes changing point of view several times in a single sentence/paragraph. But given that, I'm a little disappointed that Viv's action at the end just doesn't feel supported by all that comes before.
